Overview
All employees hired or rehired after a break in service are required to complete a 12 month probationary period. Employees transferring to VT after completing a probationary period at another state agency and without a break in service are not required to complete an additional probationary period.
Probationary Progress Reviews, 6 and 12 month
Six month and twelve month evaluations are required for all probationary employees. The P125, Probationary Progress Review, should be used for the evaluation. The original of the six and twelve month evaluations are forwarded to Human Resources.
Interim Reviews
Interim evaluations may be done at the discretion of the supervisor and should be kept in the departmental files.
Annual Evaluations for Probationary Employees
Annual evaluations are required for all employees hired prior to July 25. At the discretion of the department, the P125, Probationary Progress Review, may be used as the annual evaluation form for probationary employees.
For employees whose 6 or 12 month probationary evaluation is due between August 1 and December 1 the supervisor may complete a single evaluation form. HR will record the rating on that single evaluation for both the probationary review and the annual evaluation.
The file below illustrates the P125 schedule of evaluations due based on hire date:

Examples
- If an employee is hired in September the supervisor will complete a 6 month evaluation in March. In the fall the supervisor will complete a single evaluation form. The rating on that form will be used by HR as both the 12 month probationary rating and the annual review rating.
- If an employee is hired in November, the supervisor will complete a 6 month evaluation in May. In the fall the supervisor will complete an evaluation to meet the deadline for the annual evaluation. The rating on that form will be used by HR as both the 12 month probationary rating and the annual review rating.
- If an employee is hired in March the 6 month probationary evaluation will be due in September. The supervisor should complete a single evaluation form in the fall. The rating on that form will be used by HR as both the 6 month probationary rating and the annual review rating.
- If an employee is hired in May, an evaluation must be completed in the fall for the annual performance rating. HR will use the rating on that evaluation as both the 6 month probationary review rating (due in November) and the annual evaluation rating. In November there will be no need to do a probationary review UNLESS the employee’s performance has changed significantly from the rating completed earlier in the fall.
- If an employee is hired in January the supervisor will complete a 6 month probationary review in July. In the fall, the supervisor will have to submit a performance evaluation document to HR for the annual performance evaluation rating. If the employee’s performance has not changed significantly from the evaluation done in July, the supervisor may note on the evaluation form submitted in the fall “see P125 completed DATE”. The supervisor will then complete a 12 month probationary review in January and an annual review the following fall.
- If an employee is hired in December, the supervisor will complete a 6 month probationary review in June. In the fall, the supervisor will have to submit a performance evaluation document to HR for the annual performance evaluation rating. The 12 month probationary review will be due in December. If the employee’s performance has not changed significantly from the evaluation done in the fall, the supervisor may note on the evaluation form submitted in December “see P125 completed DATE”.
