P112 - Performance Plan and Evaluation (non-supervisory employees)
This form is to be used for all "Staff", non-supervisory employees. Use this form to establish the Performance Plan and document the employee Evaluation for the performance review cycle that begins October 2008 and ends October 2009. This form replaces the old P112-PPE.

P112S - Performance Plan and Evaluation for Supervisors
This form is to be used for all Supervisors who are "Staff" employees. "Supervisor" in this case is defined as individuals who are responsible for writing and conducting performance plans and evaluations for one or more Staff employees; OR, those who supervise one or more 1500 hour wage employees and have responsibility for interviewing, selection, hiring, coaching, disciplinary actions, etc. Use this form to establish the Performance Plan and document the employee Evaluation for the performance review cycle that begins October 2008 and ends October 2009. This form also replaces the old P112-PPE.

This file contains the competency ratings and definitions used to complete the "Job Specific Competencies" section of the Performance Plan and Evaluation (PPE). Select 3 - 5 competencies, then copy and paste into the PPE to complete the "Job Specific Competencies" section of the document.

 

P141 - Acknowledgment of Model Performance

This form is required for a Classified staff employee (hired prior to July 1, 2006 who has not elected to convert to University staff status) to be eligible for an overall Model Performance rating on the annual performance evaluation conducted in the same performance cycle.  PLEASE NOTE:  Receipt of one or more Model Performance Acknowledgement forms does not automatically entitle an employee to an overall Model Performance rating.  

 

P142 - Notice of Improvement Needed Unsatisfactory Performance 

This form (or a written notice) is required BEFORE a University or Classified staff employee can be given an overall Unacceptable Performance rating on the annual performance evaluation.  PLEASE NOTE:  Before a P142 or written notice is issued to an employee, the Supervisor must work in partnership with their local Human Resources Consultant and the Employee Relations team in central Human Resources.  If you have questions, please contact Employee Relations at 540-231-9331.
