General Procedures:

  • You MUST inform your supervisor or someone in your department of your injury immediately. If you visit a doctor your department must complete an  Employer's Accident Report, Form VWC No. 3 (rev 12/27/01),and submit it along with the Panel Selection Form and a physician completed Physical Capabilities Form to BENEFITS, Southgate Center Drive (0318), Blacksburg, VA  24061.
  • Have the physician or the hospital submit the bill to: Managed Care Innovations, Workers' Compensation Department, P. O. Box 1140, Richmond, VA  23218-1121.

Virginia Law Requires the following:

  • An Employer's Accident Report from your employer - Such reports must to be filed within 5 days of notification of job-related illnesses or injuries.
  • A choice of physicians from the above listing of the Workers' Compensation Panel of Physicians
  • An attending Physician's Report from your panel physician.
